Responsibilities and Duties Provide personal care (i.
e.
, bathing, grooming, dressing, oral care, toileting, and exercise) for patients daily and as needed.
Perform personal care services in patients' homes and assist with tasks of daily living in accordance with an established care plan.
Assist with promoting a compassionate physical and emotional environment for our patients.
Change bed linen and maintain appropriate grooming supplies in patient room on a regular basis.
Record patient vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
Observe patients carefully and observe changes in condition to Registered Nurse.
Basic food preparation, shopping, housekeeping, laundry and other errands on behalf of patient.
Be alert for proper ventilation, temperature, light and noise control.
Routinely turn and position patients, as necessary.
Provide adequate hydration to patients at regular intervals each day according to specific patient needs.
Back-up care (or respite) services:
providing other caregivers a break.
Administer simple medications.
Assist patients with mobility and physical therapy exercises.
Provide companionship and basic emotional or psychological support.
